I know this is probably redundant, but the *best* documentation there is
for HTML is the HTML 2.0 Specification, which I always get off the menu
from Yahoo in Computers:World-Wide-Web:HTML. There are also many good
style guides, but one of the best online is at www.willamette.edu, and is
most easily found as one of the items near the top of www.netscape.com's
Creating Websites page as the Composing Good HTML style guide.
Print-wise, there are several manuals appearing -- for the money,
O'Reilly and Associates book _Managing Internet Information Services_ is
a good bet, because it's got a section on HTML, including forms, but also
covers many of the kinds of services you might want to interface HTML
with, such as CGI scripts, WAIS, etc.
